Start with three questions.
What are we counting?
The metric definition sheet records the unit, numerator, denominator, source, filters, observation period, time zone, and join key. Mark a denominator “not applicable” for a count rather than inventing a rate.
What could explain the difference?
The reconciliation log records discrepancies, candidate explanations, evidence locations, unresolved questions, and the owner of the next check. Keep an explanation tentative until the evidence supports it.
What can we tell leadership?
The decision-summary template separates the answer, confidence and its basis, limitations, and recommended next action. Include the sources and period so someone else can check your conclusion.
A worked example you can inspect.
The workbook includes eight invented status events and five appointment records. Formulas count the events associated with each appointment and show the three additional occurrences. The result explains the raw difference without claiming that a scheduled appointment means care was completed.
Blank templates are separate from the worked example. Copy them before entering your own definitions and evidence. The sample formulas cover only the supplied example rows; adapt and validate ranges for your own analysis.
